Southwest General Health Center is here to support you in your wellness journey.

We do this by partnering with Southwest General Wellness, a wellness provider that works to implement wellness services in our community. Southwest General Wellness equips you with tools and resources throughout the year to guide you in Southwest General Health Center’s voluntary wellness program. The more you participate in your own health and wellbeing, the more incentives you can earn in the program.

Southwest General Wellness is a separate provider committed to your privacy and confidentiality. Your protected health information will not be shared with your employer. In addition, Southwest General Wellness fully complies with the Affordable Care Act, federal regulations and policies, and will administer compliant wellness programming.

Southwest General Wellness encourages you to feel your best. Throughout the year, you will find opportunities to take advantage of wellness resources that promote awareness and help you take charge of and maintain your wellbeing. 

Know Your Pathways

Southwest General Wellness Pathways encourage participants to live a healthy life by forging their own unique pathway to optimal wellbeing. This year, benefit-eligible employees and covered spouses will have options to qualify for incentives on healthcare in 2025.

Pathway 1

  • Complete yearly annual well visit with primary care provider by December 1, 2024 + submit screening form. Annual well visit must include height, weight, blood pressure, cholesterol, and glucose.

Pathway 2

  • Complete yearly annual well visit with primary care provider by December 1, 2024 + submit screening form. Annual well visit must include height, weight, blood pressure, cholesterol, and glucose
  • Complete 2 preventive care activities by December 1, 2024. See following page and FAQ in this guide for more details
What Else Should I Know?

This year, participants will not need to meet any wellness goals in order to earn incentives in the SWG Wellness Program. Participants can earn incentives by completing an annual well visit with their primary care provider (Pathway 1) and completing at least two preventive care activities (Pathway 2).

Completing Pathway 2 will earn the greatest amount of incentives toward healthcare costs in 2025. All portions of Pathway 2 must be completed in order to earn the incentive.

All annual well visits and preventive care activities completed for credit in the 2024 Southwest General Wellness Program should be completed between December 1, 2023 and December 1, 2024.
No onsite screenings will take place in 2024.

How To Participate

Participation in the Southwest General Wellness Program is as easy as 1, 2, 3! By participating, you’ll make strides in your wellbeing journey and earn incentives on your healthcare costs. Make sure to complete all your steps by December 1, 2024 to earn the maximum incentive. Participation is voluntary, but eligible employees and spouses who do not participate will not earn incentives towards healthcare costs in 2025.

1. Create Your Account on the NEW SWG Wellness Portal
  • Follow the instructions on the following pages to set up or log into your account; sync your wearable fitness trackers or devices; set your personal goals to customize your portal experience; and complete the Personal Health Assessment for a closer look into your current health.
2. Complete Your Screening
  • In 2024, all participants in the Southwest General Wellness Program will be asked to screen with their primary care providers by December 1, 2024. You can find the 2024 Physician Screening Form in this guide and on the Southwest General Wellness Portal. You can also review past screening results at any time on the Southwest General Wellness Portal by clicking on the “My Results” tab.
3. Complete Your Preventive Care
  • This year, participants in the SWG Wellness Program will be asked to complete a well visit with their PCP and preventive care exams by December 1, 2024 to earn the maximum incentive. You can find a list of preventive care exams on page 6 in this guide. Any documentation should be submitted to SWG Wellness by December 31, 2024.

New Users/First Time Logging in

Type the SWG Wellness Portal URL into the address bar of a new browser and hit “enter.”

Click the “Sign Up” button at the bottom of the screen.

Complete Verification Data

  • – First Name, Last Name
  • Choose an email address
  • Enter your date of birth
  • ZIP CODE: Your home ZIP code
  • Enter Employee ID number
    Spouses: Enter Employee ID number + S
    Example: EE number: 123456, SP number: 123456s
  • Click “Verify”

Create your username and password according to the prompts. Click “Create Account.”

Existing Users

Type the SWG Wellness Portal URL into the address bar of a new browser and enter your username and password. Click “Log In.”

Forms should be dated between December 1, 2023, and December 1, 2024 for credit in the 2024 Southwest General Wellness Program. Results will be uploaded on a weekly basis. Biometric data will be collected for reporting purposes and incentives. Protected Health Information will not be shared with your employer. Forms may be found on the SWG Wellness Portal and on the following page. Onsite wellness screenings will not take place this year.

Employee Assistance Program

Ease@Work
When you find yourself in need of some professional support to deal with personal, work, financial or family issues, EASE@ Work, your Employee Assistance Program, can assist.

You and your immediate family (spouse, dependent children, parents and parents-in-law) can use this program for a variety of issues, including:

  • Marital matters
  • Job related difficulties
  • Stress, anxiety and depression
  • Parent and child relationships
  • Legal and financial counseling
  • Substance abuse
  • Financial planning
  • Eldercare issues

Ease@Work has enhanced their portal and services available to include benefits such as Medical Advocacy, Life Coaching, and your very own Personal Assistant! In addition to these new services you can now have all of your EAP information at your fingertips through the My Life Expert mobile app and www.mylifeexpert.com portal. With these enhancements you will need to register on Ease@Work’s new My Life Expert portal to access your EAP benefits. Instructions to register are below:

  • Visit www.mylifeexpert.com
  • Click “Create a new account with your company code” on the right-hand side of the page.
    •  Use your company’s access code to get started
