Diabetes Management Program

Enroll in Medical Mutual’s Diabetes Management Program, and you will pay $0 toward free medications and supplies if obtained at Southwest’s pharmacy for active participation in coaching. Individual support and free supplies, sign me up! Learn more here.

Prevention & Management

Diabetes is a chronic health condition in which the body has trouble breaking down the sugar (glucose) this is in your food.  If you have Prediabetes, you are at high risk for Type 2 Diabetes (the most common type of diabetes)  and other serious health problems like heart disease, stroke, blindness or kidney failure.
